Health Insurance

Our medical fees fall within the prevailing guidelines for this area. We are a health care provider for all Medicare, Medicaid plans as well as most major insurance companies. Patients covered under these plans will render the co-pay, co-insurance amount or deductible at the time of their visit. Patients not covered will pay in full at the time of service. We accept most major credit cards. As a courtesy to our patients, we will be happy to file your claim for you.  To serve you better, please advise us of any changes in your personal information or insurance coverage. Please be sure to fill out your Patient Information Form as completely as possible. Include your insurance subscriber number as well as their correct name and address. If your insurance company pays us directly, we will credit your account and it will
be reflected in your next statement. If your insurance pays you, please promptly mail your check to us and we will credit your account.

Charges and Billing

As a convenience, those patients who have an existing balance will receive a monthly statement. Medicare patients will not receive a statement until Medicare pays their portion. You are ultimately responsible for reimbursement for medical services provided. If you have any questions or concerns regarding your account or claim, please advise one of our staff members and you will be put in touch with your account representative.
